Six posts a week from a practitioner with no time to write them

The client

Sarah Schembri runs Structure & Self, a physiotherapy and manual therapy practice working out of Beyond Health in Naxxar. One practitioner, a full clinic diary, and a growing Instagram account that patients found her through.

The problem

Content that works for a clinic has to be consistent, on-brand and clinically careful, and it has to keep coming. Six posts a week is a part-time job on its own. Sarah had the expertise and the taste, and no hours. Generic AI captions were off-brand and, worse, occasionally said things a clinician can't say in public.

The system

A fixed pillar rotation gives every day of the week a purpose, so the account never drifts into random posting:

Topics live in a Supabase table, tagged by pillar. When a pillar runs short, a live web search tops it up, and each topic is marked used once it ships, so nothing repeats. Each week, one run drafts all six posts in the practice's own structure: caption, creative direction, carousel slides, supporting Stories, hashtags, and a Facebook adaptation.

Carousel images are generated in the brand's palette (deep olive, warm cream, soft terracotta, ochre) and its photo style: a single warm raking light, anatomy-forward crops with faces out of frame, visible skin texture. Each image is graded before it's accepted.

Then every post goes to a Telegram bot. Sarah approves or rejects from her phone, usually in one sitting. Approved posts schedule through Buffer to Instagram and Facebook.

The guardrails

The rule that made the system trustworthy for a clinician: clinical safety is applied to every post automatically, not left to a reviewer to remember. Hedged language rather than promises. No numeric thresholds. No diagnosis claims. An individual-assessment note on anything that could be read as advice. It's written into the system, so a post that breaks it doesn't get drafted in the first place.

Results

The system is live and running weekly. Reach, engagement and time-saved figures from the first full quarter, and Sarah's own words, will be published here once they're in.

What's reusable

The shape of it transfers to any business that needs a steady stream of careful content: a pillar rotation, a topic bank, drafting in the house structure, guardrails built in, an approval step where the owner already is, and scheduling through whatever they publish with. The clinical rule becomes a compliance rule, a brand rule, or a legal rule depending on the business.
